New Patient Registration

blood_testNew patients wishing to register with the practice must fulfill the following criteria:

  • Live within the Practice Boundaries.
  • Have entitlement to NHS care.

If you meet this criteria and wish to register with the practice you will need to visit the reception in person to collect registration forms. A registration medical with a Nurse will then be arranged for you. It is essential that you attend for this appointment and should you fail to do so without prior notification we will not be able to offer you any further appointments.

You should bring your completed forms as well as 2 forms of ID (one to be photographic) with you to your registration appointment.

The practice reserves the right to refuse an application to join our practice list.

Once you have registered with the practice your medical records will be requested from your previous GP. Please note that it can take several months for us to receive your old medical records. You will also receive a medical card from the Business Services Organisation showing that you are registered with one of the doctors in the practice.

You may see any doctor within the practice not just the doctor you are officially registered with.
